[Bug Report: Regression in code generation quality and increased bug introduction rate

Bug Description
this was a pathetic response in the past 3 days. So many new bugs introduced, coding without checking with me, and so many bugs created. Something has changed in the last few days. I am really disappointed. Will be switching to codex and openai. Will cancel subscription. Very bad Claude Code.
