Terminal title set to bare version number — should include "Claude Code"

When Claude Code runs, it sets the terminal title via OSC escape sequences to just the version number (e.g. 2.1.128). This shows up as the window/pane name in tmux and as the tab title in terminal emulators, where the bare version is uninformative without the product name.
Suggested fix: set the title to something like Claude Code v2.1.128 (matching the string shown in the application's own header) so users can identify Claude Code sessions at a glance among other tmux windows or terminal tabs.
