[Bug] Keychain credential storage leaks orphaned entries on repeated /login

Bug Description
After /login, claude doctor warns 'macOS Keychain is not writable' (-25299). Cleaning the keychain fixes it temporarily, but every subsequent /login recreates the warning and accumulates a new 'Claude Code-credentials-<hex>' entry. Found ~85 orphaned entries over 10 days.
