[Bug] Claude Code fails to correlate detected fractures with anatomical structures in radiology analysis

Bug Description
Claude Desktop rileva visivamente discontinuità compatibili con fratture su radiografie ma non riesce a identificare le strutture ossee coinvolte — conosce l'anomalia ma non l'anatomia. Questo dissocia le due competenze necessarie per una diagnosi utile.
