[Bug] /ultrareview misleading behavior: reviews only branch diffs, not full codebase

Bug Description
The Claude Code assistant guided me to create a 1-line commit and run /ultrareview, without disclosing that it only reviews branch diffs — not the ful codebase. I had saved this limited free credit for a pre-public quality review. The credit was consumed but the intended review never happened. GitHub issue: https://github.com/anthropics/claude-code/issues/54671 Request: restore the consumed ultrareview credit.
