[BUG] All tool executions failing

What's Wrong?

Problem Description:
All tool calls return [Tool result missing due to internal error], including:

• Bash
• Read
• Write
• Glob
• All other tools

Steps to Reproduce:

1. Start Claude Code
2. Execute any Bash command (e.g., git status, pwd)
3. Returns [Tool result missing due to internal error] error

What Should Happen?

Tools should execute normally and return results.

Error Messages/Logs

All tool calls fail with no detailed error message.
