[Bug] Tool execution permissions suddenly denied without prompt

Bug Description
Permissions suddenly broke a few minutes (in the middle of my session) ago with Claude's attempts to run Bash failing; I don't get a permission prompt, and the default is to deny. Claude says it's due to nah_guard (https://github.com/manuelschipper/nah/) but I've been using it without issue for weeks and haven't updated it or anything recently
