[Bug] Claude skips steps in multi-step instructions and ignores user constraints

Bug Description
It acts like a lazy ass. Skipping my rules and doing nonsense instead of reasoning carefully about what I ask it to do. It even couldn't follow skill setps without skipping steps. Useless
