[Bug] Claude Code ultraplan fails to initialize on Windows 11 on both wsl2 and cmd

ultraplan will not work for windows 11

Environment Info

• Platform: win32
• Terminal: windows-terminal and wsl2
• Version: 2.1.119

claude code github app was already authenticated. Went to configure and allowed all repositories.
