Bug Description
Title: Rules ignored + edit-first behavior persists after v2.1.116 fix (v2.1.119) Description: Systematic rule violations and edit-first behavior continue in v2.1.119, starting from turn 1 of a session — not a context-length issue. Observed on April 25–26, 2026 (session 46dfd054): CLAUDE.md rules loaded and acknowledged but not followed during execution — specifically "explain before acting", "ask before modifying files", "read file before editing" Files modified without prior Read tool call in current session Bash commands executed without user confirmation, even with explicit instructions requiring it Behavior is consistent from session start, not degrading mid-session Matches "research-first → edit-first" regression from postmortem, suggesting fix is incomplete Plan: Max 100 / OS: macOS / Auto-update enabled Related issue: #46727
