[BUG] VSCode extension: current file context pill not shown when editor and Claude Code chat are in the same window

What's Wrong?

When the Claude Code chat tab and a file editor tab are in the same VSCode window (whether in a split pane or the same tab group), the current file context pill does not appear at the bottom of the chat input.

The file context pill only appears correctly when the Claude Code chat is opened in a separate VSCode window from the editor.

What Should Happen?

The current file context pill should appear at the bottom of the Claude Code chat input regardless of whether the Claude Code tab and the editor file tab are in the same window or in separate windows.

Steps to Reproduce

1. Open any file in VSCode
2. Open the Claude Code chat panel in the same window (split pane or same tab group)
3. Observe: no file context pill is shown at the bottom of the chat input
4. Drag the Claude Code chat tab out into a new separate VSCode window
5. Observe: the file context pill now appears correctly at the bottom of the chat input

Result: File context only works when Claude Code and the editor are in separate windows.
