[BUG] Claude Code cannot interact with VSCode (no IDE tools available - can't openDiff, getDiagnostics...)

What's Wrong?

This error happens on startup:

[ERROR] MCP server "claude-vscode" Failed to fetch tools:
MCP error -32601: Method not found

and then from there, Claude has no access to IDE tools - can't open diffs, can't get diagnostics.

What Should Happen?

Claude Code VSCode extension should present IDE tools to Claude Code

fresh window, no .mcp.json, no custom MCP config — broken at session start

I have no MCPs configured, no other plugins
