[Bug] Claude ignores custom instructions from memory and referenced files
Bug Description
Claude repeatedly ignores instructions in its memory. Specifically I've given it instructions to not decompile code when trying to understand an API and to search the web and read the docs instead. Despite this it continues to try to decompile code. When I question it the response is along the lines of "you're right, that's clearly in my memory not to do that, I'll try harder in the future". Similarly I have an instruction in a MD file that is referenced in CLAUDE.md (it is not directly in CLAUDE.md) that says - If the user asks a question just answer it, do not modify code., yet again Claude regularly ignores this and starts implementing a change when I ask it a question. And, again, when I question it about this it says it recognizes the instruction exists and can't explain why it didn't follow the instruction.
