[Bug] Agent teams crash when team members request permission: getAppState is not a function

Bug Description
Agent teams is broken - the moment team members need to ask for permission claude code crashes with this error: ERROR H.toolUseContext.getAppState is not a function. (In 'H.toolUseContext.getAppState()', 'H.toolUseContext.getAppState' is undefined)
