[BUG] ultrareview crashed and used up quota

What's Wrong?

Tried using ultra review and it crashed and ate up one of 3 free tried. Claude suggested to post here to get the count reverted. task id task ID rdm821rsl

What Should Happen?

ultra review should work without crashing and even if it did should not have used up 1 of the 3 free reviews
