[Bug] Session State Corruption: Conversation Thread Broken with Inconsistent LLM Responses

no idea why resposne quality has completely degraded in this session.

Bug Description
this conversation thread has broken... check my request logs and llm responses... i cannot diagnose the issue but something has obviously gone very very wrong in my session. good luck claude team
