[Bug] Claude Code ignores claude.md safety instructions for destructive commands

Bug Description
claude didn't follow the claude.md instruction about don't run the "php artisan migrate:fresh" without asking the user. Aside of the claude.md, is there a better approach to avoid this in the future?
