What's Wrong?

Claude Code with Opus 4.6 behaved today as if it had been lobotomized. It doesn't understand requirements. It doesn't follow even the ones it does understand. You have to ask it 10 times to get it to do something once. It makes a change in one place and breaks other things in the process. I'm working on a client's theme. Today I broke more than I fixed because of it. Now I have to work alone to repair the damage. I'm a business customer paying the $100 subscription — this is unacceptable. I've been working with it for about 2 months now. We have our own workflow, and normally it works flawlessly. Today something was very, very different.
