VS Extension: Support ANSI color palette configuration for terminal

Problem Statement

In the Claude Code VS extension, the terminal's ANSI colors (especially diff output)
are very hard to read. Neither claudecode-settings.json nor VS Options > Terminal
expose any color/theme configuration. Please add ANSI color palette settings
(similar to Windows Terminal color schemes) to the extension settings.
