[FEATURE] Show current name of the session in the tab name

Problem Statement

I run multiple tabs for claude code. However, they all look same to me. I recently saw /rename feature, it ll be great if that can be suraced on top of tab name for easier nabvigations.

Proposed Solution

Whatever user sets name of session by using /rename , it can surface on top as tab name as well.
