VSCode Extension: Missing Header Button After Local Install

Bug Description
The VSCode Extension doesn't display the Run Claude Code button in the IDE header. It appears after running npm install -g @anthropic-ai/claude-code. But after the installation, I can see multiple installation problems when running Claude Doctor. Also I have an issue with CMD+Enter to create new line
