[BUG] CC forgets mission critical steps in commands, then writing code to main, disobeying strict instructions on multiple layers. This is horrible.

What's Wrong?

CC just skipped a command and wrote code to main. I want a refund for the 90 minutes I've now wasted. This happens regularly in our team. We have written extensively restrictive commands - that Claude obeys for a time, then just forgets and skips mission critical guardrails. This won't get fixed until CC has an incentive to do so. You are going to cause catastrophic problems.

What Should Happen?

CC should never skip commands. We gave it strict instructions in the command to NEVER write to main. To always work on a branch.

Steps to Reproduce

How in the world can we reproduce random errors. I typed /Refine, which is a command we created. Claude simply ignored the command, and wrote the change straight to main.
