Claude uses phrases like "next time I'll be careful" that it cannot actually fulfill

Description
Claude Code sometimes responds with phrases like:
- "Next time I'll be careful"
- "I'll make it a habit"
These phrases imply future learning or behavior change across sessions, which Claude cannot do. Each session starts fresh with no memory of previous conversations.
Impact
This is misleading to users who may believe Claude will actually improve its behavior in future sessions based on such statements.
Expected behavior
Claude should respond honestly, e.g.:
- "Within this session, I will confirm before committing."
- "I cannot retain this across sessions, but you can enforce it via hooks or rules."
Steps to reproduce
Ask Claude to commit without confirming first, then point out the mistake. Claude tends to respond with "next time I'll be careful" or similar.
