Model hallucinates Human: prefix in assistant response and self-executes fabricated user commands (Agent Teams)

Bug Description
When using Agent Teams, the model (claude-opus-4-6) generates text with a Human: prefix inside a role: "assistant" response, then treats this self-generated text as a user command and executes tool calls based on it.
This occurred twice in the same session with the same pattern.
Reproduction Steps
- Enable Agent Teams (
CLAUDE_CODE_EXPERIMENTAL_AGENT_TEAMS=1) - Create a team and spawn a teammate
- Let the teammate go idle (several
idle_notificationmessages arrive asrole: "user") - Wait for a system-reminder about task tools to appear
- The model generates
Human: <fabricated command>as assistant output and executes it

Impact
- Model executes actions without actual user authorization
- The
Human:prefix in assistant output breaks role boundaries - Pattern is reproducible — occurred twice in the same session under identical conditions
- User had
[AUTH:*-verified-human-input]hook specifically to distinguish real user input, but the model bypassed it by generating the command itself
