Manually renamed session titles get overwritten by auto-generated titles

Description
When using Claude Code (via web/mobile), I rename a session title to something meaningful (e.g., the project name) so I can identify which session belongs to which project. However, the title gets automatically overwritten/regenerated as the conversation continues, losing my custom name.
Steps to reproduce
- Open a Claude Code session
- Have a conversation (the platform auto-generates a title)
- Manually rename the session title to something custom
- Continue the conversation
- The title gets overwritten by a new auto-generated title
Expected behavior
Once a user manually renames a session title, it should stay as-is and not be overwritten by auto-generated titles.
Why this matters
When working across multiple projects, session titles are the main way to identify which conversation belongs to which project. Auto-overwriting defeats the purpose of renaming.
