Voice mode: content loss and push-to-talk UX issues

Problems
Three issues with voice mode (/voice):
1. Content loss — beginning of dictation disappears
Spoke for ~5 minutes. The beginning of the dictation disappeared from the chat log. Only the tail end was captured. The text was visibly transcribing during dictation but was gone by the time it was submitted.
2. Content loss — max length cutoff without warning
The dictation also hit some maximum and cut off the end. No warning that the limit was approaching.
3. Push-to-talk is the only option
Having to hold the space bar (or a key) for the entire duration of a long dictation is ergonomically bad. A toggle mode (press once to start, press again to stop) or hands-free mode would be much more usable for longer inputs.
Expected behavior
- All dictated content should be preserved in the chat
- If there's a max length, warn before hitting it, or better, don't have one
- Offer a toggle/hands-free mode as an alternative to push-to-talk
Environment
- Claude Code CLI on macOS (Darwin 24.6.0)
- Voice mode enabled via /voice
- Default space bar push-to-talk
