Terminal scrollback cleared at random during conversation

Terminal scrollback is being wiped at random points during a conversation. After it happens, only the startup banner is visible (model name, version, working directory). Conversation context is preserved — Claude still knows what we were discussing — but all visual output is gone and scrollback is empty (can't scroll up to recover it).

• Model: Opus 4.6 (1M context)

No hooks configured. No shell auto-clear. Doesn't correlate with a specific action — seems random. Not auto-compaction (context window was at 4% when investigated).
