[Bug] Degraded Claude Code reasoning and execution quality within sessions

Bug Description
esta cada dia pior de usar o claude code. é perceptivel a queda na qualidade de raciocio e execusão. todo dia eu começo as interações e o claude responde a altura e monta e executa tudo conforme as orientacoes e num momento ele começa a errar umas bobeiras e fica impossivel de resolver. eu estou pagando um valor bem consideravel de assinatira mas nao estou recebendo o produto. quero reportar uma reclamacao formalizada sobre isso. é inaceitavel.
