[BUG] All slash commands broken in v2.1.89 ("Unknown skill" error)

What's Wrong?

After upgrading from v2.1.87 to v2.1.89, all slash commands (/pm, /commit,
etc.) fail with the error:

Unknown skill: <command_name>

Steps to Reproduce

  1. Upgrade Claude Code from v2.1.87 to v2.1.89
  2. Run any slash command (e.g. /pm, /commit)
  3. Observe "Unknown skill" error

Environment

  • Claude Code version: 2.1.89 (broken), 2.1.87 (working)
  • OS: macOS Darwin 23.3.0
  • Shell: zsh

What Should Happen?

Slash commands execute normally (as they do in v2.1.87).
