[Bug] Fallback logic violates CLAUDE.md explicit prohibition on automatic fallbacks

Bug Description
I have an explicit instruction in CLAUDE.md about fallbacks:
- Do NOT add fallbacks automatically. If you think one is needed explain the reasoning and ask for permission to use fallback. Fallbacks are in general FORBIDDEN in this code base!
Despite of this Claude Code still adds absolutely useless fallbacks which are basically bombs waiting to explode during testing and production.
