[FEATURE] Allow renaming conversation tabs in VS Code extension

Problem Statement

Currently, conversation tabs are auto-named from the first message and cannot be renamed. When working with multiple conversations, the truncated auto-names make it hard to distinguish between tabs.

Request: Allow users to rename/edit conversation tab titles (e.g., right-click → Rename, or double-click the tab title).
