[Bug] /feedback command still failing — "Error submitting feedback / bug report" (regression, March 2026)

Bug Description
The /feedback command fails with "Error submitting feedback / bug report" — still reproducible in March 2026.

This was previously reported in #11013 (closed as "not planned") and #12638 (closed as duplicate), but the bug persists in the current version.

Steps to Reproduce

1. Run /feedback inside Claude Code
2. Type any feedback text
3. Submit

Expected: feedback submitted successfully
Actual: "Error submitting feedback / bug report"

Environment

• Platform: macOS (MacBook M4)
• Claude Code version: 2.1.81
• Date: March 31, 2026
• Occurrences today: 2

Note
Issues #11013 and #12638 were closed without a fix. Given the bug is still present 5 months later, please reconsider the priority.
