[Bug] Claude Code making unauthorized modifications to unrelated files

Bug Description
In the last two days, Claude Code is consistently making unauthorized changes to files where no prompt or direction was given to touch them. [Image #5] This is just one example, and it's becoming a pattern now and extremely concerning and breaking trust. I will submit more screenshots with feedback as I continue seeing this happen.
