Verbose transcript view (Ctrl+O) doesn't auto-refresh while thinking

Description

When Claude Code is processing in the background (thinking/running tools), the verbose transcript view toggled with Ctrl+O doesn't auto-update with new content. You have to toggle it off and back on to see new output.

Expected behavior

The verbose transcript view should stream/auto-refresh as new content arrives while Claude is working in the background, similar to how the main output streams.

Current behavior

  1. Claude starts working on a task (thinking/running tools in background)
  2. Press Ctrl+O to see what's happening
  3. The view shows content up to that moment but doesn't update
  4. Have to press Ctrl+O twice (off then on) to refresh and see new content
