Plan mode deactivates after using bypass permission

Bug Description

When plan mode is active and the user uses bypass permission (to approve a tool call), plan mode is unexpectedly deactivated. The user has to re-enter plan mode manually.

Steps to Reproduce

  1. Enter plan mode (e.g. via /plan or by typing "plan mode")
  2. Claude proposes a tool call that requires permission
  3. User selects "bypass" / approves the permission
  4. Plan mode is no longer active — Claude proceeds as if in normal mode

Expected Behaviour

Plan mode should remain active after bypassing a permission prompt. The permission bypass should only affect the specific tool call, not the overall mode.

  • Claude Code CLI
