[BUG] Diff stats indicator (+N -N) no longer shown after file edits, only 'Modified' or 'Added N lines' (but no diff)

Preflight Checklist
- [x] I have searched existing issues and this hasn't been reported yet
- [x] This is a single bug report (please file separate reports for different bugs)
- [x] I am using the latest version of Claude Code
What's Wrong?
In the Claude tab in VSCode, changes are no longer displayed as diff, just 'Modified' or eg 'Added 6 lines'.
But these are not clickable, and a diff is not available.
Clicking the file name will just open the file an mark the affected paragraph, not a diff.
What Should Happen?
There used to be a clickable line '+N, -N' opening up a diff.
