[BUG] Read tool prompts for permission unexpectedly

What's Wrong?

Read calls to ~/.claude/CLAUDE.md trigger an "Allow Claude to Read" permission prompt, which shouldn't happen per the docs ("Read-only tools do not require approval")

What Should Happen?

Claude should read the doc without prompting for permission.
