What's Wrong?

I encountered a critical issue where Claud Code automatically modified my sources files without my permission while I set to 'Plan Mode'

What Should Happen?

Claude should follow the setting values, and it shouldn't modify my personal file without my persmission.

Steps to Reproduce

1. Set Cloud Code to 'Plan Mode'.
2. Provide a prompt or instruction that requires code changes.
3. Observe that the tool applies the changes directly to the files instead of just presenting a plan.
