[Bug] Plan mode bypassed: agent modifies code without creating plan first

Bug Description
I'm currently on plan mode and the agent still updated code without creating a plan first. I accpeted the changes becuase they were correct but there was no initial plan. also, it commited and pushed the changes (I accpeted) without me prompting it.
