Voice mode stops working in existing terminals, only works in new ones

Description

Voice mode stops working in already-open terminal sessions but continues to work in newly opened terminals. Initially it was working in all terminals, then at some point it stopped responding in the existing ones — only new terminals work.

Steps to reproduce

1. Open multiple terminals with Claude Code
2. Use voice mode across them (works fine initially)
3. After some time, voice mode stops responding in the older terminals
4. Open a new terminal — voice mode works there

Expected behavior

Voice mode should continue working in all terminal sessions, not just newly opened ones.
