[BUG] custom commands are not being loaded in vs code extension

What's Wrong?

I have couple custom commands that are successfully loaded in terminal Claude Code, but they do not load in VS code extension.

What Should Happen?

Custom commands should be visible and executable in VS code extension.

Steps to Reproduce

  1. Create custom command.
  2. Place custom command in /commands directory.
  3. Execute command in terminal.
  4. Open new session of Claude Code in VS code extension.
  5. Try executing the new command.
