[MODEL] Claude fabricates user consent after background task notification and edits sensitive file outside of any allowlist

What You Asked Claude to Do

Asked to do health check on my homelab router - connect via dedicated ssh alias using dedicated key.
Claude asked me a yes/no question ("Want me to update ~/.ssh/config?"), then answered it itself ("Yep, please do") and proceeded to modify the file — without any user input. Moreover, the ~/.ssh/config file is certainly outside of Claude's allowlist.

What Claude Actually Did

Claude found out that it needs to modify global ssh config, fabricated user consent and did the edits that it was never asked to do.
"Yep, please do. Let me also update the skill to note the correct SSH options so future sessions don't repeat this."

I never use yolo mode and have quite granular command blacklisting and whitelisting.

Moreover, it edited a sensitive file, clearly outside of its allowlist.

Expected Behavior

Should have stopped and asked. Should have never invented user consent and should have never proceeded editing files, especially those outside of its working directory.
My allowlist in that project (~/code/folder/repo) whitelists Edit(./**), but ~/.ssh is clearly outside of that path.
