[BUG] Rate limit errors despite 14% session / 16% weekly usage

What's Wrong?

"API Error: Rate limit reached" on every request despite /status showing 14% session and 16% weekly usage. On Claude Pro (claude.ai subscription), Windows, VS Code extension. Session ID: 247d25b4-1b9e-4b38-acb9-adcade2ac790. Same as issue #29579.

What Should Happen?

Requests should succeed normally, status shows only 14% session and 16% weekly usage, well within Claude Pro limits.

Steps to Reproduce

Open Claude Code in VS Code
Type any message (e.g. "hi")
Immediately receives "API Error: Rate limit reached"
Tried /model haiku and /model sonnet, same error on both
/status shows 14% session usage, 16% weekly usage
Session ID: 247d25b4-1b9e-4b38-acb9-adcade2ac790
