[BUG] plan name persists in status bar after plan is completed

What's Wrong?

When a plan is created and fully executed to completion, the plan name continues to display in the CLI status bar (top-right corner). This is misleading as it suggests the plan is still active.

What Should Happen?

Expected: Plan indicator clears after execution completes.

Steps to Reproduce

1. Enter plan mode and create a plan
2. Exit plan mode with user approval
3. Execute all steps to completion
4. Plan name still shows in status bar
