[BUG] Plan mode ignores user instructions after plan creation

What's Wrong?

In Claude Code website, Plan mode behavior has changed. Previously, when I provided alternative instructions instead of approving a plan, Claude would revise the plan accordingly. Now, Claude treats any response as plan approval and proceeds with the original plan, ignoring my alternative instructions.

What Should Happen?

Claude should revise the plan based on new instructions, not treat them as approval

1. Use Claude Code in Plan mode on website
2. Let Claude create a plan
3. Instead of approving, provide different instructions
4. Claude ignores new instructions and executes original plan
