Title: Claude Code becomes unresponsive after prompts - no response or action taken

Description:
I'm experiencing a bug where Claude Code frequently doesn't respond or take any action after I enter a prompt. This is new behavior - Claude Code worked properly before this update.

Steps to Reproduce:

1. Start a conversation with Claude Code
2. Enter a prompt requesting work on a task
3. Claude Code either doesn't respond at all or stops mid-task without completing the requested work

Expected Behavior:
Claude Code should respond to every prompt and complete requested tasks without stopping unexpectedly

Actual Behavior:

• Multiple instances of no response after entering prompts
• Claude stops working mid-task without explanation
• Have to re-prompt multiple times to get a response

Environment:

• Claude Code (Anthropic's official CLI)
• Started noticing issue: [today's date]

Additional Context:
This appears to be a regression - Claude Code was working fine in previous sessions. The issue is affecting productivity as I have to repeatedly re-prompt to get responses.
