[Bug] Claude Code ignores user rules and constraints across contexts despite acknowledging feedback

Bug Description
Claude Code keeps saying "let me save this as feedback so it doesn't happen again" and yet it CONSTANTLY happens again. Why it is incapable of following clearly stated rules even when it's running in a fresh context?
