[BUG] session transcript: `custom-title` and `agent-name` entries keep added even when there is no change in the session title

What's Wrong?

Transcript entries with type custom-title and agent-name are added redundantly for no practical reason, seemingly

What Should Happen?

If the session name is unchanged, don't add more entries for those.

Steps to Reproduce

1. Start a session
2. Rename the session
3. Exit and enter the session multiple times
4. Redundant rename-related entries are added to the transcript
