/rename breaks /resume picker — shows only renamed session

Bug

After using /rename to set a custom session title, the /resume picker inside the same session shows only 1 session (the renamed one) instead of all available sessions.

Steps to reproduce

1. Have multiple sessions (e.g. 48)
2. Inside a running session, run /rename some-name
3. Run /resume in the same session
4. Only the renamed session appears in the picker

Expected behavior

/resume should show all sessions, same as before renaming.

Workaround

claude --resume from outside (new terminal) shows all sessions correctly. The bug is only in the in-session /resume UI.
