[Bug] Anthropic API Error: Rate limit exceeded with low usage quota

Bug Description
API Error: Rate limit reached in Claude Code despite 0% 5-hour usage and 6% weekly usage on Claude Pro. Persists across re-logins and npm cleanup. Running v2.1.81 on Windows.
